PACE is a program of all-inclusive care for the elderly. Our mission at PACE is to enhance life for older adults with chonic needs by providing quality care and services. We enmpower participants to live in their homes and in the community for as long as it is medically and socially safe, provide comprehensive care that promotes independence and the highest levels of functioning, while allowing choice and dignity for members and their families. Participant eligibility for PACE is as follows; Participants must be age 55 or older, reside in a PACE service area, meet nursing home level of care through LTSS screening, and be able to safely live in the community with the support of PACE. PACE provides services through an interdisciplinary team approach and provides to all participants primary care, medication management and prescription medications, appointment scheduling and care coordination, transportation to all medical appointments and to and from the day center (wheelchair transport available), home care and aide services, rehabilitation and therapy including PT/OT/ST, adult day care, social services and mental health supports, nutritional services and dietary consult, durable medical equipment, respite services, 24-7 on-call physician services, personal emergency response, vision, hearing and dental services. Living at home is your goal and our priority.
